Your Rights
You have the right to receive equal treatment regardless of class, income, race/ethnicity, religion, national origin, age, gender, marital/legal status, sexual orientation, gender identity or expression, disability, Veteran status, physical size and genetic information.
You have the right to receive services professionally in a comfortable, safe environment.
You have the right to be treated with respect by all members of the organization.
You have the right to actively participate in decisions about the services you receive.
You have the right to have your information and records kept private and confidential.
You have the right to be heard regarding your satisfaction with the services you receive, and to with the Program Director if you are not satisfied with services.
You are provided with the opportunity to complete a survey about expressing your opinion and make comments regarding the services you received.
Your Responsibilities
You have the responsibility to treat staff members with respect.
You have the responsibility of providing necessary and accurate information necessary to allow staff to provide the best available services based on your individual circumstances.
You have the responsibility to provide at least 24 hours’ notice, or as much time as possible, if you need to cancel or reschedule an appointment.
